1. Definitions
   • Sunway Medical Centre Sdn Bhd, a company registered in Malaysia under company
     registration number 341855-X
   • SunMed Kid’s Club membership card is issued by Sunway Medical Centre
   • Card Holder means all the holders of a valid SunMed Kid’s Club membership card
2. The SunMed Kid’s Club membership card is a privilege card which gives Card Holders the benefits of enjoying discounts, privileges, events from time to time offered by Sunway Medical Centre
3. The SunMed Kid’s Club membership card is non-transferable and non-assignable and is only for the exclusive use of the Card Holder only
4. The SunMed Kid’s Club membership card is NOT a point accumulation card. It does not allow or require Card Holder to collect points for gift exchange or similar purposes
5. The SunMed Kid’s Club membership card is NOT a credit card, debit card or the like
6. Discount privileges are valid only upon presentation of your SunMed Kid’s Club membership card during registration. No reimbursement or refund on previous or back-dated transactions if card holders fail to present the SunMed Kid’s Club membership card during registration
7. Discount privileges strictly apply to self-pay cases only
8. Unless otherwise stated, the card cannot be used in conjunction with other cards and/or other promotions
9. Sunway Medical Centre reserves the right to discontinue, modify or add any terms and conditions, benefits or privileges granted by this card as deemed necessary without prior notice
10. Sunway Medical Centre is not liable for any loss or damage incurred by the Card Holders due to any unauthorized use of the SunMed Kid’s Club membership card
11. Any request for replacement of SunMed Kid’s Club membership card for any reason whatsoever including but not limited to loss or damaged cards, shall be subjected to a fee of RM10 per card
12. Should any of the conditions and privileges are misused or abused, Sunway Medical Centre is entitled to suspend and terminate the Card Holder’s right to use the SunMed Kid’s Club membership card without prior notification
13. Sunway Medical Centre undertakes that all personal information supplied by the Card Holder for the registration of the SunMed Kid’s Club membership card shall not be disclosed to any third party unless with the prior written consent of the Card Holders or required by law
14. The SunMed Kid’s Club membership card benefit and privileges is redeemable after a period of 7 working days from the date of successful registration
15. The SunMed Kid's Club membership card is applicable to Malaysian citizens and Permanent Residents (PR) from birth up to 12 years old.

Explicit Consent Clause

By submitting a registration form, I have read the Personal Data Protection Notice provided by Sunway Medical Centre Sdn Bhd pursuant to Section 7 of the Personal Data Protection Act 2010

(www.sunwaymedical.com), which includes purposes for which my personal data and sensitive personal data are collected / processed and classes of third parties to whom Sunway Medical Centre Sdn Bhd will / may disclose my personal data and sensitive personal data to.

I hereby give consent to Sunway Medical Centre to process my personal data and sensitive personal data in accordance with the Personal Data Protection Notice.
