1. Valid for patients aged 6 months to 18 years old only.
2. This package is valid until 31 December 2025.
3. This package is available by appointment only.
4. The package is for self-pay patients only. It is not applicable for insurance claims or corporate billing.
5. The package only covers the tests and items listed below and any additional tests will result in charges to be incurred accordingly:
   1. Iron (FE)
   2. Full blood count
   3. Peripheral blood film
   4. HB electrophoresis/HB analysis
   5. Ferritin
   6. Consultation with the participating consultant
   7. Registration fees
6. This package is only applicable when seen by the participating consultant listed in the package.
7. This package does not cover follow-up appointments.
8. This package applies to outpatient services only.
9. Sunway Medical Centre reserves the right to amend these terms and conditions without prior notice.
10. This offer is not applicable for any emergency cases, out of office hours and complications.
