A Milestone Achievement at Sunway Medical Centre, Sunway City

The first Endoscopic Ultrasound-guided Hepaticogastrostomy (EUS-HGS) at Sunway Medical Centre, Sunway City, performed by Dr Khoo Stanley, took place in early October 2025.

What Is EUS-HGS?

EUS-HGS is an advanced endoscopic procedure that combines endoscopy and ultrasound to create a new drainage pathway between the liver’s bile duct and the stomach. Using a gastroscope equipped with an ultrasound probe, the doctor visualises the bile ducts through the stomach wall and accesses the bile duct with a fine needle. A biliary stent is then placed to connect the bile duct to the stomach, allowing bile to drain directly into the stomach. The procedure is performed under general anaesthesia in a fluoroscopy suite.

Indications for EUS-HGS

EUS-HGS is usually performed when bile cannot flow normally due to bile duct obstruction, which is predominantly caused by:

Malignant biliary obstruction – pancreatic cancer, liver cancer or cholangiocarcinoma
Benign biliary obstruction – gallstones, bile duct narrowing (stricture), or chronic pancreatitis
Post-surgical anatomy anomalies – after bile duct surgeries
Failed Endoscopic Retrograde Cholangiopancreatography (ERCP)
